What is MI Care Manager?

MI Care Manager is a private owned agency aimed at assisting Older Adults -residing in Wayne, Oakland and Macomb County, Michigan- with sorting through and responding to the physical and social changes that occur as a person ages. The owner, Ebone’ C. Jolly, is a Registered Nurse, based in Southfield, Michigan with several years’ experience ensuring quality care for seniors with various needs.    

MI Care Manager was founded with the intent of helping Older Adults access and navigate the vast web of services necessary for maintaining an optimal quality of life. The services provided, often reduce or even eliminate the confusion and frustration that comes with researching, selecting and managing the endless maze of options competing for attention.  

The mission of MI Care Manager is to provide individualized support through coaching, advocacy and coordination of personal services on behalf of Older Adults. The servicer’s commitment to excellence is motivated by a passion for improving lives and evident through positive relationships and ongoing professional development. 


MI Care Manager’s vision is to provide the highest quality Care Management service, by taking a proactive approach to addressing the health and disability, financial, housing, family dynamics, resource acquisition, legal and crisis intervention needs of elderly adults in support of the mission.
